Tips for submitting your application:

Before you begin the SIMS application, please gather all required information including your application essay and Curriculum Vitae (PDF format, only). Application questions may take up to one hour to complete and a partially completed application cannot be saved for later.

Your essay should be one page typed (about 500 words) and describe one or more of the following:

  • In your own words, explain the definition of addiction.
  • How has addiction personally affected your own life?
  • What do you hope to achieve by participating in the SIMS program?

In addition to completing the application form, two letters of recommendation are required (PDF format, only). While it is preferred that letters of recommendation be submitted as part of the online process, these two items may be submitted after the online application is complete. Submit letters of recommendation in PDF format via email to MPE@HazeldenBettyFord.org, or in print format by postal mail to:

Joseph Skrajewski
Executive Director of Medical and Professional Education
Hazelden Betty Ford Foundation, Daniels Building
39000 Bob Hope Drive
Rancho Mirage, CA 92270

Letters of recommendation must be received by either the priority application date or final application date, and your application is not complete without them.

We will begin to fill the limited number of program seats immediately following the PRIORITY application deadline on February 1, 2017. To be considered for a remaining seat, if any, applications will be accepted until March 1, 2017.

No applications will be accepted after the final deadline. Applicants and alternates invited to participate in the SIMS program will be notified by phone by April 1, 2017. Applicants who are not selected will be notified by email by April 8, 2017.
