The Preferred Provider level of the Patient Care Network is comprised of organizations who, together, form our high-quality national provider network designed to improve care for patients and families impacted by addiction. The Patient Care Network Preferred Provider mission is to strengthen the continuity of care for patients by providing a seamless transition from one level of care to another, or from one provider type to another. The patient experience will be enhanced by improving collaboration, communication and integration of care teams working together to support individual needs. Together, through stronger partnerships, we will ultimately improve patient outcomes and help more individuals find freedom from addiction.
