Our continuum of care, which includes inpatient, outpatient, recovery housing and recovery management, provides your clients the appropriate level of care to meet their clinical needs. Over 80% of our adult patients either remain continuously abstinent or dramatically reduce their use in the year after treatment. Patients with an opioid use disorder who are in our opioid treatment program stay longer, discharge atypically less frequently and have better abstinence rates than those not in our opioid treatment program. We're more than a 28-day program. Addiction is a chronic condition that requires continuing care. Hazelden Betty Ford Foundation offers a full range of services with varying intensities and durations to meet the clinical needs of each individual. Each patient is assigned a highly-trained, multidisciplinary treatment team. Hazelden Betty Ford Foundation employs on-site medical doctors, psychiatrists, psychologists, nurses, counselors and case managers in residential programs in California, Oregon and Minnesota who collaborate to assess and adjust each individual's treatment plan. Other programs will have combination of these disciplines as well based on the level of care provided. We provide FREE recovery support to patients once they complete treatment. Research shows that clients who actively engage in continuing care after treatment are more likely to remain abstinent. Hazelden Betty Ford Foundation's exclusive MORE (My Ongoing Recovery Experience) program keeps patients connected by providing state-of-the art Web-based tools, support and fellowship to build a new life in recovery. Following treatment, each patient receives continued guidance from a recovery coach (a licensed addiction counselor) both electronically and by phone to provide encouragement and support. In addition, we offer patients and their families the Connection program. Connection services incorporate the most promising recovery support practices, including: intensive personal coaching and case management frequent phone contact with a recovery coach—for patients and designated family members documented recovery compliance updates random drug testing for accountability The Connection team will lead patients through three distinct phases based on emerging research about what makes recovery last: an initial focus on practical tasks: developing their support network, managing their time, following through with commitments ongoing identification of roadblocks related to their social, emotional, spiritual, financial and vocational well-being personal goal-setting and creating an action plan for reaching those goals Hazelden Betty Ford Foundation is insurance friendly. We are an in-network provider for many insurance companies. Our Financial Case Managers work with your clients and their insurance company to determine a financial plan that will make treatment possible. For those who qualify, patient aid is available.
